The certificate is dated 1st May 1919 and is a printed document by Alan Taylor, Illuminator of Manchester, with the personal details added. Sergeant Cheadle was wounded at Ypres in 1918 and had a leg amputated in February 1945 as a result of the wounds. He died on 30th November 1981.
